Some food safety systems, like BRC do require a glass policy. You need to
read in detail the full requirements of whatever standard you are meeting.
The latest BRC std- Version 5- has more than just a policy required.
To meet the requirements to meet Section 4.8.4 BRC version 5 you need the
following:
- A (written) risk assessment to identify the potential for glass
contamination
- control measures in place to eliminate, reduce, minimize glass
contamination
- Documented (i.e. written) procedures for handling including
cleaning and replacement for not just glass but also: brittle and hard
plastics and ceramics.
- You need a written list of all glass, brittle and hard plastics
and ceramics on site including their location, number, type and condition
and how often you are going to check them.
- Documented procedures for glass breakage including quarantining
the products and production area, cleaning the area, changing clothes of
staff involved act as per 4.8.4.3
- Records of glass inspections and glass breakages.
You really need to have a copy of whatever standard/code of practice/
legislation you are trying to meet in your hand and not someone else's
version of that document. So you know exactly what is expected long before
an audit.

The wording 'glass policy' is not part of, and can not be found in the EU
Hygiene Regulations, but 'glass' must of course be explicidly addressed in
the PRP & HACCP study.
Addressing the hazard of contaminating the product by glass is mandatory:
1) in the PRP: glass from (wrist) watches, drinking bottles/cups, jewelry,
devices, clocks, windows,
2) in HACCP: where glass is used as primary packaging.
What is described below is part of some Food (safety) Standards (like BRC).

A glass policy shall be written and implemented. The policy shall
state that no glass is to be used in the facility, except where absolutely
necessary (like eyewear, MIG thermometer). The policy shall also state
that no glass shall be brought into the facility in employees' personal
effects. Included in the policy shall be a procedure for handling any
glass that is broken in the facility. In addition, a list of all essential
glass should be compiled and the items on the list checked on a regular
basis to ensure that any accidental breakage is noted.
The policy is same whether it is EU or USFDA. Its a part of pre-requisite
program.
