Question about Hazard Analysis

    Could someone help me with this.
    I am looking for a reference of bi-dimensional grid to determine "Significant hazards" based
    on the Severity and Likelihood of Occurrence (Low and High levels or Low-Medum-High)
    I found a reference (Codex Alimentarius Risk Assessment Grid) in

    which mention it but it does not give details about this "simplified version" (Low-High levels).
    I want to determine where did it come from, who proposed it or issued it. How useful is it?
    This tool is tool is not intended for use in seafood HACCP.
