For students who will be applying to the ADA’s Accredited Dietetic
Internship Programs in February, 2006, please note the following deadlines
and information:
* Form B needs to go to the people who will be writing letters of
recommendation for you no later than Friday, December 16th. While this
form is typically used for faculty in the Nutrition Department, feel free
to give a copy to letter writers who are located outside the
department. Form B and instructions are located online at
<http://nutrition.ucdavis.edu/ada.html>http://nutrition.ucdavis.edu/ada.html.
* If the person writing your letters is not located in the Nutrition
Department, you should be sure to give them the Waiver & Recommendation
form (under the Non-Departmental Recommendation Forms Heading – same page).
* Form A is due on the first day of Winter Quarter instruction –
January 4th. Be sure to include a check made out to UC Regents in the
amount of $50.00.
* Confidentiality statements (located on the same web page, under the
Departmental Forms heading) should be given to the Advising office along
with your Form A. You need one statement for each place you are applying,
times the number of faculty from the Nutrition department who are writing
you letters.
* The D&D Forms will be given to you when you receive your packet of
materials from the advising office. The packet of materials will
include: D&D Forms, Letters of Recommendation (individually addressed,
sealed and signed), a Declaration of Intent or Verification Statement, and
a checklist of the classes used to complete your individual Didactic
Program in Dietetics (DPD). These will be ready for you prior to the
mailing deadline.
