I don't have my Gregg's desk reference here...but for what this is worth,
in my military past, it was proper to address retired officers with the
highest rank they held, such as "general." I don't think you can go
wrong by bestowing this honor on a former member of the bench. If you
are incorrect, the judge will correct you! Of course, if the judge loses
an election, and returns to her former law practice, then she probably
would insist on being called "attorney Smith." If you don't get a better
response, you might refer to your state's Canons of Judicial conduct, or
the Model Rules. Hope this helps.
