Locating forms of agreement

I'm trying to locate a form often referred to as a "private label agreement"
or 'brand label agreement" in which one party manufactures a product
which is sold under another's party label. The agreement covers issues
of warranty, product liability, etc. I've searched Am Jur Legal Forms,
Rabkin and Johnson, and Nichols Cyclopedia of Legal Forms without
success. Can anyone recommend a source (print, online or internet)?
