Thanks to everyone who responded. The majority of the responses favored approval by a practice area leader or managing partner. Several allowed the librarian to make purchases up to approx. $200. I now feel more confident in making recommendations on a new policy.

Good Morning!!
I'm trying to get my hands on HCFA form 855, Medicare and other federal
health care program general enrollment, dated 1/98. It's about 17 pages.
I have the Medicare/Medicaid Regulatory CD, but the form will not print out
for some reason (and I've tried all the usual threats and fixes to try to
get it to work).
If you guys can help, I'd appreciate it. We'll pay for faxing or shipping
fees.
