I nominate the Office of the Federal Register
which prints a replacement cover for a CFR that is not being republished. (e.g.
 32 CFR Parts 630 - 699, 1998)

Only because I've studied Japanese orgami am I able to fold it fit squarely on
the old volume. Who cares that it takes 15 minutes.
