I believe that the new edition of the Columbia Gazetteer of the World will do just that. I had an opportunity to examine the new edition at SLA in June. It looks like they did an excellent job. Check out their website at http://www.columbia.edu/cu/cup/gazetteer/

Rand McNally formerly published a nifty little green book called The
Rand McNally Green Guide: The Reliable Source to U.S. Places of Over
100 People. It allowed one to easily identify the county that a
particular city was in. Unfortunately, Rand McNally does not publish
this any more. Does anyone know of a comparable source? TIA
