We use a book entitled Directory of Cities and Counties, Counties and
County Seats of the United States, published by

AM Publication

It is not as detailed as your green book ("places of over 100 people")
but it is pretty comprehensive. Alternatively, you can always use the
U.S. Postal Service address server <http://www.usps.gov/ncsc/> if you
have an exact address -- just pop in the information, and it will
usually come back with the county. Be aware, however, that there have
been problems with this usage in the past where an address was on the
border between two counties, especially when the address is located in a
large MSA such as Atlanta. I suspect that the Postal Service may have
been notified about the problem, given that the server will not process
counties for some given addresses now.

I have been told that the only definitive way of determining county
jurisdiction is by calling the Sheriff's office or the county tax
collector, but I have even found this to be unreliable! I would
appreciate learning of any other sources my colleagues may have for use
in these circumstances.

     Rand McNally formerly published a nifty little green book called
The
     Rand McNally Green Guide: The Reliable Source to U.S. Places of
Over
     100 People. It allowed one to easily identify the county that a
     particular city was in. Unfortunately, Rand McNally does not
publish
     this any more. Does anyone know of a comparable source? TIA
