Check out the CSC website. Their forms can be filled out and printed
online. Charges are incurred only if you have CSC do the processing.
Below is their most recent e-mail about the service.

What's the most efficient way to prepare UCC filings in one or more
jurisdictions? INCSPOT.

Access the new UCC filing preparation feature on IncSpot - available
exclusively to IncSpot members.

Now, you can prepare UCC filings in one or many jurisdictions quickly and
accurately. You can also access real-time updates on your UCC filings and
receive notification via E-mail that your UCC documents have been filed.

The question and answer format helps guide you through the process of
preparing your UCC filings. With the UCC filing preparation feature, just
enter the UCC filing information once. The information you enter is placed
on
each UCC form for the selected jurisdictions.

To use the UCC filing preparation feature, you will need Netscape Navigator
3.01 or higher or Internet Explorer 4.0 or higher. Adobe Acrobat Reader is
also needed to print the completed UCC forms. Adobe Acrobat can be
downloaded
free of charge from the Adobe Web site (www.adobe.com) or by clicking on the

"Get Acrobat Reader" icon on IncSpot.

Use the UCC filing preparation feature now through September 11, 1998, and
incur no fees for preparing your UCC documents. (Note: CSC service fees for
filing still apply.) To access the UCC feature, log on to IncSpot at
http://www.incspot.com, and enter your username and password in the Client
Spot
section. Then, look for "Filing Preparation" on the left-hand menu bar. It's

that easy!
