The "Notification & Report form for Certain Mergers & Acquisitions" is
in fact on the FTC web site under Antitrust/Pre-Merger/Hart-Scott etc..
The problem is that C4 refers to the instructions and the current web
version (which is not dated) does not use that designation (although
the instructions printed in the CFR does use C4.) The instructions are
a text file but the form itself is a pdf file.
I would be very careful about using information that is not currently
found or traceable to a website; especially government sites. Sometimes
sections of government sites are copied (not linked) to other sites and
the information is not updated. These sections can then turn up out of
context, so to speak.
BTW by just putting in "form" in the FTC search engine the M&A form came
up.
