I've pulled the form off our IHS Medicaid/Medicare Library CD-Rom (I
didn't see it on our CCH). There wasn't a Form 885 but Form 855 that is
entitled Medicare Provider/Supplier Enrollment Application so I thought
perhaps that was the one you were looking for. If that sounds correct,
let me know and I will see if I can find the instructions on the CD that
go with it before I fax it all to you. -Connie
