UCC Standard Forms

I am trying to help out the person in our office who handles supplies and
forms. She stocks a Standard UCC form for UCC-1 that comes from a local
forms supplier. One of the attorneys showed her a similar form that has many
more boxes than the standard. It appears to have a 5/82 date on it. The
attorneys wants only this form, not the standard that we stock. We have shown
this 5/82 form to our supplier, but they don't seem to have it.
The current form has 4 numbered boxes. The 5/82 form has 10 numbered boxes.

Does anyone understand what I am talking about? I think there has been a
change in the UCC. Did this change also alter the standard UCC-1?

Sorry to be so vague and unclear, but I am not sure where to go to find out if
the change in the UCC altered the forms in the way that I described.
