A TNT article stated that this form would be on the IRS home page at the end
of last week. I have checked it several times and can't find it. I have
also checked several of our print sources and have not been able to locate a
copy of this new form. Our local irs office does not have it either. If
you have a copy of this form, would you please fax it to me.
