I need help obtaining a copy of the following for a partner:

Occupationa Safety and Health Administration.
This is exactly how the cite appears at the end of an article in
a corporate in-house newsletter. I was given only the page with the
references.
If any kind soul out there has this CD-ROM and could send me this
letter, I'd be most appreciative. We will pay any costs incurred.
