Hello,
I'm looking for a form that we believe is in Hospital Legal Forms
Checklist and Guidelines, Rowland and Rowland published by Aspen.
I've tried all the local sources, and an OCLC search shows the
closest copy to be 300 miles away. We need a form that deals with
consent for tissue donation. If that's not available, then a form on
tissue and organ donation consent will suffice. If someone could
please fax me the form(s), I would be very grateful.
