Thanks for the info. One can get to look at and copy these
forms at public inspection rooms. Best to use someone you know
in DC. My apologies to Kristen in SSD Washington office. I
should have realized she would be an expert in the government

Our technical rep seems to think this was a Columbus Ohio
problem, but responses so far indicate that this is a wide
