I am hoping that somebody will be able to fax/fedex/or even mail me
copies of the Sample Demand Letters from the Medicare Carriers Manual
section 7130. They were included in Trans. 1510 (Feb 1995). I have
checked OCLC and called everybody in town listed as having it plus a
few others, but nobody has the February 1995 transmittal. The CCH
Medicare Medicaid Guide par. 11,245 refers to the sample letters, but
does not include them. NTIS will only sell me the entire set for
close to $200.
Any help will be appreciated. We will of course pay any charges.
