    I'm trying to track down a copy of the original Revised Long-Term Care
    Facility Resident Assessment Instrument User's Manual, Version 2.0 (RAI
    Manual). The RAI Manual is a publication of the federal Department of
    Health and Human Services (DHHS) Centers for Medicare & Medicaid
    Services (CMS). The version 2.0 manual was published in December 2002,
    and has since undergone a number of minor revisions. The current
    version, along with revision notes, are available at
    http://www.cms.hhs.gov/NursingHomeQualityInits/20_NHQIMDS20.asp. But, I
    haven't been able to find an original copy of the 2002 manual without
    any of the revisions. I have tried WorldCat with no luck.

    Any help finding this or providing a copy if you can would be
    appreciated.
