Many thanks to those who pointed out a couple of serious flaws with the
initial survey setup. I have fixed the problem, but unfortunately as a
result lost the responses of 12 participants who filled out the survey
in the first 5-10 minutes. There is a problem with one other question
that may affect some others, but I do not want to risk losing responses
again so we will leave it as is.
If anyone was an early responder to the survey (thanks!) and would like
