My firm is working on improving our New Business Form, which is the form we have the attorneys & secretaries fill out for conflict checking/accounting when they open a matter for either a new or existing client.
So I have been tasked with rounding up samples. If you can supply a blank copy of the form your firm is using, it would be greatly appreciated.
