    I'm hoping someone can find something along these lines. I've been
    trying to find a form that a patron can use as an example. She has a
    daughter who is presumed dead, but nothing official has ever been done.
    She was told by the state department of unclaimed property that she
    needs to give them this form to get the property released. I've had no
    luck finding anything along these lines.

    Does anyone have something along these lines they can send? I would be
    eternally grateful!
