    Disease management: a systematic process of managing care of patients with
    specific diseases or conditions (particularly chronic conditions) across the
    spectrum of outpatient, inpatient, and ancillary services. The purposes of
    disease management may include: reduce acute episodes, reduce
    hospitalizations, reduce variations in care, improve health outcomes, and
    reduce costs. Disease management may involve continuous quality improvement
    or other management paradigms. It may involve a cyclical process of
    following practice protocols, measuring the resulting outcomes, feeding
    those results back to clinicians, and revising protocols as appropriate.

    Good morning. I am in search of medical dictionary definitions for the terms
    "case management" and "disease management. I've been able to find "case
    management" in the 28th edition of the PDR dictionary. I also spoke with
    Westlaw, who has a dictionary source called Stedmans that also had a
    definition for "case management."
     
    However, I am not finding any definitions for "disease management."
    Currently I'm checking with Lexis to see if they have any other dictionaries
    available, but any other suggestions or recommendations would be much
    appreciated.
