Also, the front page of transmittals are available from MediRegs. If you need the actual pages that were to be filed you can sometimes get the entire transmittal from NTIS.

I am looking for the following citations:
 
HCFA Transmittal No. 64, State Medicaid Manual, § 3257(B)(6) and § 3258.10(B)(1)
 
I think it is from Nov. 1994.
 
I have searched Westlaw and CCH Internet Research Network plus Google and Yahoo. I can find numerous references to the document but not the document itself. I would gladly accept any advice on how to find the entire Transmittal 64 and/or my specified sections.
