Drafting a collection survey

I work for a mid-size private law firm (100+ attorneys), and I am in the
process of drafting a survey to gauge and compare usage stats and
research habits with reference to select print and electronic resources
in our collection. I have some idea of how to approach this, but I want
to keep it short and sweet without overlooking important variables.
 
If you have experience in formulating effective library surveys and
would like to share any insights or suggestions, I would love to hear
from you.
