Does anyone have, or know a source for, a BLANK SEC Form 8-A that would
have been in effect in December, 1995?
I have the release that amended the form in 1994, and it contains the
requirements, the language, etc., and so have what the form should
contain--and the FR says that the form was not published in the CFR--but
the attorney is just sure there is a blank form out there somewhere.
None of the major publishers nor the SEC has it, so I'm hoping someone
out there has a file of them.
If you have one, or can offer any advice, please let me know.
