Hello,

Our library is in the process of revising librarian evaluations and I am
seeking your assistance.

If any of you have good assessment forms and/or procedures for the
upward review of supervisors, could you please send me a copy of the
form, and/or explanation of your procedure?

Please send responses directly to me and I will be happy to summarize
them (anonymously) for the list.
