Dear Catherine:

I am sorry to be late in responding to your query. I know you already
have several sources for the answer, but I can't resist mentioning a
favorite print source in many law libraries' Reference collections:

CSG State Directory. Directory III Administrative Officials/ Lexington,
KY.: The Council, c1998-. JK 2403 .S711

Directory III (there are three others, one for legislative and one for
elective officials) groups state government agencies by topic (from
Agriculture to Workers Compensation). Within each topic, name, address,
phone, fax and email contact information is provided for each of the 50
states. There is a two+ page listing of all the state tax departments
under "Revenue".

Hope this helps at least for future reference. Better late than
never.:-)
