Good afternoon all -
I'm looking for accompanying forms to: Multifamily Accelerated
Processing (MAP) Guide, which HUD says have not been updated since 2000.
My OCLC searches reveal that several folk have the Guide, but no mention
of the forms. I've left a message at our local MAP office, but haven't
heard back from the supervisor and the staff there have never heard of
the "Forms Book." See the attached (in the small print at the bottom)
for the mention of the forms, "The FHA and HUD Forms referenced in the
Guide and set forth in the Forms Book which accompanies the Guide are
approved as...."
As you can see from the following text from the HUD web site, the
revised MAP Guide is available online, but not the forms.
Does anyone have the Form Book and would be willing to loan it to us for
a short while?

The Multifamily Accelerated Processing (MAP) Guide, originally
published May 17, 2000, was revised March 15, 2002. The list of MAP
Chapters, Appendices, and other text below are linked to this recent
revision. Additionally, the links below titled "Transmittal" and "New
Changes to the MAP Guide" will provide an explanation of the changes
included in this March 15, 2002 revision. The MAP Guide is approved for
use by MAP-approved Lenders and by HUD Multifamily Offices.
The MAP Guide is approximately 565 pages. You may view or download the
