Kirkland & Ellis publishes a book of wills and trusts forms. There used
to be a printed version, but I think they may have gone to disk only
now. You could give them a call. As I recall, it was the Chicago
office.

We subscribe to multiple estate planning titles. Some have forms mixed
in with the rules/treatises. I don't know that any of these would match
your exact needs, but you might check their publishers for other titles:
Estate Plannin & Taxation Coordinator from Thomson-RIA
Tax Management Portfolios: Estates, Gifts, and Trusts from BNA US Tax
Reporter: Estate and Gift Taxes from Thomson-RIA
All of these titles are huge multi-volume sets that are updated
frequently with looseleaf, so this may not be what you're looking for.

One of our attorneys is looking for a book which includes various trust
forms (as in estate & trust). Phone calls to various large publishers
yielded no positive results. Either because the book isn't out there or
because the publishers are so big, they don't know what they own..
