Many people have been contacting me about this since I posted a request
for it in Nov. 2002. We did find it. It is in the Auditor's Letter
Handbook (1990) published by the ABA. Copies are for sale at the ABA's

It is cited in Sec. 2.3 of the ABA's Model Rules of Professional Conduct
