The answer is Bloomberg. I am also attempting to verify the information with the Japan Company Handbook. If you can read Japanese you won't have the problems finding this that I did.
Summary of responses:
1. Some of this information may be found in the annual report, but chances are the
report is in Japanese. If you let me know the name of the company or companies
you are looking for, I can tell you what we have. Alternatively, you can go to
www.global-reports.com and browse through the Japanese index there.
2. We use Thomson a lot for the foreign filings. What is the company name?

3. I suspect it is in the official Japanese filing called the Yuho for short.
Unfortunately it is only published in Japanese.

5. I don't know where to find the answer but some ideas / leads come to mind:

6. Teikoku is file 502 on Dialog.
7. I thought it might be a longshot but their Web site (Teikoku) says they do custom research
on markets. Perhaps they can extend that service to fit your needs or give you an
idea where to find the info. I notice MaryDee mentions Dialog but I had already
checked the Dialog file and it doesn't contain shareholder names. This could take
research within the country/native language.
8. As prescribed by Japanese Securities and Exchange Law, shareholders with more than 5% stake in public companies are suppose file statements with local finance bureaus and the securuties exchange houses. I don't know if there is a database in which you can do a search to get this information. Good luck!
9. I would try Perfect Information <http://www.perfectinfo.com/frontend/home_frameset.htm> http://www.perfectinfo.com/frontend/home_frameset.htm, they are based in London but have a NY office. We use them for international company filings and they do have a research service. The phone number for customer service I have is 1-866-845-2080. Unfortunately your filings will probably be in Japanese! :)
10. Perhaps, the Japan Company Handbook would give you the information.
The Japan Company Handbook publishes Japanese company information that includes no of shareholders, and Major holders. If
you like, I can search your company, if you give me the name.

11. I am not sure if it has the sort of information you are seeking, nore for
that matter wether they are still produced but there used to be a quarterly
publication entitled the Japan Company Handbook.
Sorry I cannot give you any further details but they used to provide a page
of details on japanese companies.
12. Shareworld from Thomson Financial is probably your best bet. They collect shareholder information from a variety of sources for Japanese companies too. Japanese public companies are required to disclose their top ten Shareholders in the annual report (yuho) filing. Shareworld also contains this data.
As someone else mentioned on the list already, the Japan Company Handbook does have some shareholder information but it only lists the top ten shareholders for each company. It is published by the Toyo Keizai company. Toyo Keizai also does a survey of the top 30 shareholders for each public company in Japan, though not all companies (such as Sony) provide the full 30. You would have to buy the data (bilingual) directly from Toyo Keizai as they don't release it elsewhere.
Japanese companies are also required to make a filing when anyone own 5% or more of their stock. Unfortunately, to my knolwedge, this filing is only available from a somewhat obscure database in Japanese.
