California Advance Health Care Directive might be worth looking at. It's
part of the probate code (sections 4700-470?).

Are there any formal requirements (acknowledgement/notarization)
to authenicate a "living will" to preclude artificial,medical life-support?

Martindale Hubbell's Law Digest has nothing on this subject.

Looks like CA repealed its "Natural Death Act" in July of 2000.

Prior CA case law gives anyone the right to refuse such treatment.

What is the day to day practice these days?
