Or production/rendering issues. When situations occur where appendices are referred to in the text, but don't exist in the book, or where an index goes from A-P (period), I think of production/rendering issues.

It doesn't seem like someone who would gravitate toward an editorial position would drop off whole appendices without noticing the slips; or stop an index at "P". It seems more likely in such situations that a couple of files didn't get transferred to the production/rendering team.

In another title (Am Jur Trials) the new index refers to vols. that
haven't been published yet. I guess they *are* having editorial
difficulties.
