I did a "search this site" of the EEOC web site for "employer information
report" and found a sample of the form and its instruction. It is
stamped SAMPLE.
It's at:

From the EEOC home page you would choose Statistics, then Job Patterns for
Minorities and Women in Private Industry. There's a link in the text
to EEO-1 reports.
