citing jury instructions

Does anyone have a feel for the proper citation of jury instructions? I
can't find any examples in the Bluebook. The closest rule seems to be
12.8.5. The patron has about 10 different examples of instructions he is
citing as a means of comparison. I suggested the patron consult the "cite
this work as" statement in front of the books but unfortunately he has
collected jury instructions from several different states and has only
copies of the instructions, not copies of the actual books or title pages.
(We do not keep instructions from other states). So given this lack of
information, I am suggesting he follow rule 12.8.5 and include the title of
the book (in small caps) with the jury instruction number and the date.
(leaving out the responsible institution and the title of the instruction).
Any comments on whether this is approach is acceptable?
