I'm not sure if there is a seperate instructions for this (I could be
wrong). I think the last time this came up, we had to use instructions for
Part IV of schedule A to form 990 as a guide.

I'm trying to locate the instructions for IRS Form 8734. We do not have it
in the binder in our collection and I have not been able to come up with it
locally as of yet and it was not on the IRS website. I did find the form at
our local County Law Library but they did not have the instructions either.
If anyone would happen to have it, we would appreciate obtaining it via fax
