One of our attorney's is trying to track down an example of some forms used
in sweepstakes (I don't think this is a legal form per se but we're not
sure) The two forms he needs are an "affidavit of eligibility release" and
"liability release" forms, these are listed in sweepstakes as necessary to
fill out to win.
I would be happy with any direction any of you could provide, we've already
tried Lexis, Westlaw and the Internet.
