For those who are interested, forms can be downloaded from the Federal Securities Law Reporter (on Lexis or from CCH's online service). Or, Proformware
www.proformware.com has software to do the job.
Finally, I was pointed to the Securities Division web site for Washington State, which has forms. They are PDF files, but are editable within the browser.
