Thanks everyone, for your tips on the 4/7/00 FR. I'd already checked the sudocs website and couldn't find a scanned page, but I'll try it again.
Also, the CCH Medicare & Medicaid Guide reprinted it last week, as part of Release 1103 (part 2), dated 4/18/00, as actual FR pages (pretty tiny print, but it can be enlarged upon copying).
This whole escapade brings up a pet peeve. Is anyone else finding that increasingly, locating the material on-line isn't good enough for many users/patrons/attorneys? This was the second instance just this week where our library staff produced on-line versions of material (1 was this FR, the other was a newspaper article), but the requesting attorney wanted photocopies of the original, printed version. What gives?
