We are trying to find a copy of the Pennsylvania form for Durable Power
of Attorney (under code section 20-5604). I have tried the Purdon's and
various Internet sites for forms. We believe there may be a model form
in Chapter 69 of Dunlap-Hanna's Pennsylvania Forms. Would someone be
willing to fax me a copy of the Pennsylvania DPOA form from Dunlap-Hanna
or some other source? Thanks.
