I do not have the forms, but in reading Goergia Code section 19-9-23 (Action to
obtain change of legal custody...), it seems this is done by filing a
complaint/petition. Here is what 19-9-23 (a) prescribes ".......after a court
has determine who is to be the legal custodian of a child, any COMPLAINTseeking
to obtain a change of legal custody of the child should be brought as a seperate
action in the county of residence of the legal custodian of the child".
Hopefully, this will be helpful to you.

I represent a client in Arizona who is seeking to challene custody of her
children who reside in Georgia. In Arizona, West Practice Forms Domestic
Violense suggest an "Order to Show Cause" why custody should remain with
custodial parent. However, not having access to Georgia's domestic forms, I am
wondering if there is something similar. Any direction and assistance is greatly
appreciated.
