"For anyone who uses West's Oregon Rules of Court:State (2000)"
there is an error in the address of the Oregon Workers' Compensation
Board at the bottom of page 250. The board moved 6 years ago, so the
correct address is:

                Oregon Workers' Compensation Board
                2250 McGilchrist S.E.
                Salem, Oregon 97302-1282
